How do I get a copy of my medical records?

You can get access to all your medical records by logging in to our patient portal, using your user id and password. Click the button below to learn more about the portal.

You can also visit our office to request copies of your medical records. There is a charge of $25 to fill out the FMLA paperwork.

Is there a charge for copying my medical records?

We do not charge for patients for copy of their records. We do charge $25.00 to send a disc/CD.

There are no copy fees if records are sent directly from our office to another healthcare provider.
